Nurse Practitioner needed in Northern California Clinics

BHRs partner Medical Group is seeking a sharp, self driven, organized and creative California Licensed Nurse Practitioner or Clinical Nurse Specialist with a proven track record to provide clinical services in our outpatient Medication Clinics in Oakland and Union City. Our clinics are also staffed with Psychiatrists and Licensed Therapists who provide services to HMO, Private Pay and County patients.

FULL-TIME:    Oakland and Union City Clinic 
v      Full-time positions - will be an employee of Medical Group
v      40 hours per week
v      Weekdays (some weekend and evening shifts)

v      Clinics are open 8:30am – 5pm Monday – Saturday; Evenings by appointment only.

SALARY RANGES$55-$65 per hour + excellent benefit package
Salary would depend on:
v      Experience & license category and specialty

QUALIFICATIONS:

v      Relevant medical experience in Mental Health

v      Current California Nurse Practitioner License and DEA Number

v      Leadership experience  

v      Excellent communication/presentation skills 

v      Superior documentation skills

v      Basic computer skills and an aptitude for learning new computer applications for          

         clinical practice

RESPONSIBILITIES:

 

  • Primary job duties will be to treat patients in an Outpatient Medical Clinic setting.
  • Work independently and under direct supervision of the doctors.
  • Treatment and Case Management
    - Take history of the current complaint and conduct initial evaluation/assessment
    - Formulate the assessment of the current problem
    - Formulate the disposition and/or therapeutic treatment plan, including ordering and interpreting diagnostic testing and providing patient education
    - Provide follow-up to patients in a timely manner
    - Identify patients in need of therapy services and refer them to the in-house therapist as appropriate
    - Establish communication with consulting psychiatrists, therapists, and families for case management and coordination of care for patients
  • Documentation of Practice
    - Assure that all reports required by clinic are complete, accurate, and prepared in a timely manner
    - Maintain legible, concise, and accurate medical records of patient care in an appropriate format
    - Maintain accurate patient tracking systems for medical screening programs
    - Document case management activities within medical records
  • Teaching and Education

- Provide leadership and training to lower level staf 
- Integrate patient education regarding mental health maintenance
- Maintain an appropriate and up-to-date file of patient education handouts
