| QUALIFICATIONS: |

- BS degree minimum, Masters Degree preferred
- Registered Nurse with current valid license in California and current CPR card.
- At least two (2) years experience working with clients with serious and persistent mental illness.
- In-patient setting, as well as experience in an outpatient medical setting.
- Experience in providing superior physician coordination of client services.
- Excellent group facilitation skills and experience.
- Strong knowledge base and understanding of psychiatric diagnoses, terminology and medical record charting.
- Utilization Review and Management Skills.
- Excellent communication skills, verbal and written
- Ability to work well with minimal supervision
- Ability to work well with physicians, clinicians and other nurses.
- Ability to handle multiple demands and/or crisis situations in a calm, professional manner.
- Excellent interpersonal and client engagement skills and experience.
- Knowledge and understanding of managing record charting.

| GENERAL RESPONSIBILITIES: |

- Document daily contacts with others involved in the client’s care (i.e. case managers, ACCESS, family members, residential care providers, guardians,
hospitals discharge planners, etc.)
- For all new clients, monitor client participation in treatment and design a client engagement system for immediate follow up in the event a client misses a
scheduled appointment.
- Assist in obtaining insurance cards; processing TARs; maintaining housing; and providing a list of resources for clients in need of these services. If working
with children/adolescents, this also includes making contact with schools, courts, family services, etc. as needed.
- On a daily basis, check each chart of clients seen that day to ensure that physicians, and clients have signed and completed all pertinent forms (medical
consent forms; client treatment plan, etc.)
- Work closely with Psychiatrists on discharge planning for assigned client.
- Complete required records documentation daily/weekly /annually and keep medical records and clinical documentation current at all times. All notes must be
legible.
- Attend staff meetings and communicate clinical concerns to team.
- Participate in Continuous Quality Improvement Management team programs as requested by the Medical Director. (i.e. distribute client satisfaction survey to
each client at each visit to the clinic, monitor how many clients are being hospitalized each month, etc.)
- Participate in special/emergency cases, case conferences, and client staffings as required.
- Provide all clients with care, dignity and respect -- continuously striving to deliver the highest level of support and client-centered treatment.
- Perform other related duties as requested.
