| QUALIFICATIONS: |

- Masters Degree minimum
- Registered Nurse and Clinical Nurse Practitioner current valid license in California, and current CPR card.
- At least two (2) years experience working with clients with serious and persistent mental illness.
- In-patient setting, as well as experience in an outpatient medical setting.
- Experience in providing superior physician coordination of client services.
- Excellent group facilitation skills and experience.
- Strong knowledge base and understanding of psychiatric diagnoses, terminology and medical record charting.
- Utilization Review and Management Skills.
- Excellent communication skills, verbal and written
- Ability to work well with minimal supervision
- Ability to work well with physicians, clinicians and other nurses.
- Ability to handle multiple demands and/or crisis situations in a calm, professional manner.
- Excellent interpersonal and client engagement skills and experience.
- Knowledge and understanding of managing record charting.

| GENERAL RESPONSIBILITIES: |

- Conduct assessments on all new children and adult admissions. Notes physician orders and follows through as needed.
- Conducts medication check visits on returning clients as required.
- Coordinates all laboratory procedures, including Infection Control responsibilities.
- Assists with coordinating clients' scheduled appointments/sessions with physicians
- Contacts others involved in the care of clients regarding medication/medical treatment issues, as necessary. [i.e. case manager, family members, crisis staff, pediatricians, etc.
- Conducts client medication education groups and provides individual client education when necessary.
- Daily documentation and implementation of treatment plans for all clients on caseload
- Participate in Treatment Team Meetings and Clinical Trainings
- Provide all clients with care, dignity and respect. Continuously strive to deliver superior clinical and client-centered treatment.
- Perform other related duties as requested.
