STATUS

 

16 – 24 hours Clinician Positions

     -

 

 

QUALIFICATIONS:                       1.   California State Licensed Social Worker or

Marriage & Family Therapist or Psychologist

2.      Must have license for a minimum of  two (2) years and experience working with patients in mental health setting.

3.      Ability to engage and retain patients assigned to caseload.

4.      Experience in providing superior case management services to patients.

5.      Excellent therapy and group facilitation skills, and a minimum of 2 year experience as a therapist.

6.      Knowledge and understanding of psychiatric diagnoses, terminology and medical record charting.

7.      Excellent communication skills, verbal and written

8.      Ability to work well with minimal supervision

9.      Ability to work well with physicians, nurses and other clinicians.

 

GENERAL RESPONSIBILITIES:  Responsible for conducting individual, family and/or group therapy, treatment planning, documenting patient status and response to treatment, and coordinating program and case management services with other service providers.

 

SPECIFIC RESPONSIBILITIES: 

 

Licensed Clinician shall:

 

Treatment and Documentation:                       

 

 1.  Conduct assessments on new patients assigned to caseload.

 

2.      Formulate a comprehensive Treatment Plan on each new patient after initial visit.  Along with setting short-term and long-term goals, maintaining monthly records on the progress, and revising, changing or deleting plan for each new client as needed.

 

3.      Schedule at least one monthly one to one session with patients.

 

4.      Checks medical record to ensure that there is an authorization in the medical record prior to seeing patients and facilitates “reauthorization” after initial authorization visits have been used.

 

5.      Complete the required clinician documentation daily/weekly and keep medical records and clinical documentation current at all times.  All notes must legible.

 

6.      Conduct group therapy if the clinic establishes a group of patients who could benefit from group therapy.

 

7.      Attend monthly staff meetings and communicate clinical concerns to team.

 

8.      Participate in Continuous Quality Improvement programs as requested by the Operations Director.

 

9.      Participate in Clinical Trainings.

 

10.  Provide all patients with care, dignity and respect. Continuously strive to deliver superior clinical and client-centered treatment.

 

 

 

 

Case Management

 

  1. Develop therapeutic relationship with all patients on caseload, schedules regular appointment for one to one interaction to set and review patient's goals and progress in treatment.

 

  1. Document all contacts with others involved in the patient's care (i.e. case managers, care managers with HMOs, family members, guardians, hospital discharge planners, etc.)

 

  1. Monitor patient participation in treatment and set up systems to follow up as soon as patients miss a scheduled appointment. Document all missed appointments and interventions in the medical record.

 

  1. Work closely with Physicians and Nurses on discharge planning for assigned clients.

 

  1. Performs other duties as assigned by the Management Company to promote excellent patient care.

 

Schedules

50 minute sessions with patients and 10 minutes for documentation per patient per hour
