INTAKE TEAM LEADER
1. Masters Degree in Human Services or related field
2. At least 1 year experience in conducting intakes/assessments or working in a call center
3. Experience leading teams
4. Diagnostic evaluation and assessments for adults and children
5. Ability to screen new patients for outpatient service needs
6. Excellent written, verbal and follow-up skills
7. Minimum of two (2) years experience working in an mental health field
8. Solid knowledge of HIPAA regulations and guidelines.
9. Ability to create, organize and manage all intake office systems
10. Superior problem-solving skills
11. Highly proficient in knowledge of computers, Microsoft excel and able to enter high volumes of data
12. Ability to interact with SMI clients, program staff, physicians, or departments and outside agencies in a professional manner.
13. Ability to handle multiple demands in a calm, controlled manner.
14. Ability to deliver "excellent" customer service to all patients enrolled in the clinic.
15. Ability to multi-task in a high volume situation.
16. Ability to work independently

GENERAL RESPONSIBILITIES: Coordinate all incoming patient referrals and patient appointment with scheduled physicians. Triage referrals and coordinate with physicians and/or therapist schedules. Responsible for patient tracking throughout the clinic system. Enter new patient information into the computer and or updated information on clients daily. Complete initial referral intake paperwork on all new patients. Coordinate eligibility verification with billing group. Develop therapeutic relationship with all patients via phone at the time of referral. Track all incoming referrals contacts and trends. Prepare weekly monthly reports on all activities. Provide all patients and customers with dignity.
PAY SCALE: Intake Team Leader- $45,760-$52,000/year
RESPONSIBLE TO: Director of Operations
SPECIFIC RESPONSIBILITIES:
- Act as the Team Leader for Intake Department
- Verify Medi-Cal eligibility and ensure that all new county patients have episode openings assigned to them before scheduling appointments
- Verify commercial insurances eligibility/benefits via phone
- Triage referrals and coordinate with Physician's and or Therapist's schedules.
- Ability to gather and write clear, concise Intake Assessments so that the clinics have the ability to formulate and implement an effective treatment
- Ensure daily episode closing for expired referrals and re-engagement of clients
- Attend required annual Company and County trainings as requested
- Maintain and updates clinical knowledge by readings, in-service training in designated areas and other activities in consultation with Director of Operations and/or Medical Director. Must complete at least 24 hours of in-service training each year
- Coordinate interpreting services for all new patients requiring a translator
- Coordinate transportation services for all new patients requiring transportation services
- Work closely with the accounting department regarding incoming invoices related to transportation and/or translation services for all new patients
- Request collateral paperwork from County social workers or outside therapists
- Facilitate quarterly meeting with each County's ACCESS Department to communicate and ensure the smooth admissions process of all ACCESS Referrals as requested by Executive leadership team
- Ensure all reminder calls and letters for new patients are completed
- Make sure that Intake staff follow up with all new client who no-show to their initial appointments (always track “reason why” they did not attend)
- Schedule regular weekly/monthly meetings with Intake Department Staff
- Submit Monthly Intake Reports to Executive Leadership Team
- Keep Executive leadership team informed on new business opportunities
- Assist in the facilitation of centralized scheduling for all programs
- Perform other duties as assigned.
